Who this notice is for
This notice is for patients, people enquiring about care, and clinicians asking about courses or team training. It covers this website, the forms on it, and messages you send to the practice email or phone number.
What we collect
What we hold depends on how you get in touch.
- Enquiries. Your name, email, date of birth, address, telephone number, the topic you chose, your message, and how you prefer to be contacted.
- Appointment requests. The service, whether it is an initial visit or a follow-up, the location, a preferred date and time, your name, date of birth, address, postcode, email, telephone number, a short note about what is happening and how long it has been happening, any referral you choose to mention, and how you expect to fund the visit.
- Courses and team training. Your name, email, phone number if you give one, profession, organisation, the course or training need, preferred location or delivery, approximate numbers, and any message.
- Calls and emails. Whatever you tell us so we can reply.
Appointment requests can include information about your health. That is kept so Georgios can review the request. It is not copied into an ordinary notification email, and it is not shown on the public website.
If you tick the optional box on an enquiry, we also record that you are happy to receive occasional practice news by email.
Why we use it
- To reply to your enquiry or training request.
- To review an appointment request and contact you about the next step.
- To provide physiotherapy, ear care, or the course you asked about, if you go ahead.
- To deal with funding or an insurer, only where you have asked us to use that route or care requires it.
- To send occasional practice news, only if you opted in.
- To keep clinical and business records we are required to keep.
We rely on your consent when you send a form and agree to the notice. Where the information is needed to consider or provide care, we also rely on that purpose. Practice news is sent only with the optional opt-in.
What a form does not do
Sending a form does not book a time. Appointment requests are marked as awaiting review. An appointment exists only after we confirm it with you. The confirmation message on screen is an acknowledgement that we received the request.
Who else sees it
We do not sell your information and we do not use it for advertising.
- Georgios and the practice team who need it to reply or to provide care.
- An insurer you have named, such as Aviva, Bupa, Vitality, AXA, WPA or Healix, only when that is how the visit is being funded and you have asked us to deal with them.
- A service that stores the form so we can read it. The submission is not published. Clinic maps on this site are loaded from Google. Google receives the fact that the map was opened. It does not receive your form.
Clinics are at Barbourne Health Centre (44 Droitwich Road, Worcester, WR3 7LH); House of Health (90 Pope Street, Birmingham, B1 3BH); Midland Health (78-79 Francis Rd, Edgbaston, Birmingham, B16 8SP). Home and virtual appointments are arranged separately.
How long we keep it
If you become a patient, the clinical record is kept for as long as professional and legal duties require. An enquiry or course message that does not become a clinical record is kept while we need it to respond, and for a short period afterwards. You can ask how long a particular record is being kept.
Your choices
- Ask for a copy of the information we hold about you.
- Ask us to correct something that is wrong.
- Ask us to delete information, where we are allowed to.
- Ask us to limit how we use it, or object to a particular use.
- Withdraw a marketing opt-in at any time. Care and replies do not depend on that box.
- Complain to the Information Commissioner’s Office at ico.org.uk if you are unhappy with how we handle your information. Please talk to us first if you can.
Email yourcaringphysios@gmail.com or call 0330 133 3257. There is no charge for a reasonable request.
